  • A comprehensive review of your medical history, physical examination, medication review and perform diagnostic and preventive screenings**.
  • Personalized care recommendations based on your visit that are shared with you and your primary care provider, if you have one.

What should I expect from my visit?

Before your visit

Once you schedule your Healthy Home Visit,  you’ll get a confirmation for your appointment. You may also get an email, text or call reminder 24 hours before your visit. 

During your visit

On the day of your visit, a Signify Health licensed clinician will spend one-on-one time discussing your health with you in the comfort of your home. They’ll answer your health questions and more, including:

  • Checking your vital signs and reflexes
  • Reviewing your medical and family  history
  • Going over your medications
  • Performing preventative and diagnostic screenings*

After the visit

A summary of the visit will be shared with you and your primary care provider, if you have one. Signify Health may also help coordinate additional services or connect you with resources you need based on your visit.

Yes, in many regions we offer appointments seven days a week, from 7 am to as late as 10 pm. What are preventive screenings?

Your Healthy Home Visit is more than just a check-up. In some cases, based on your medical history, the visit can include valuable services to identify or monitor conditions like COPD, chronic kidney disease, diabetes, and more. These services allow you to manage your health conditions proactively, helping slow disease progression and prevent costly health complications. It's all about keeping you healthier, longer, and all in one convenient visit.

How can I find my results from a Healthy Home Visit?

You will receive a one-page summary and action plan from your licensed Signify Health clinician immediately after your visit. A full report on your visit is also shared with your primary care provider, if you have one. And, you can always reach out to Signify Health’s Member Services team by calling 855-319-448, to answer questions about your recent visit.
