Cohort Calls

 

We are excited to begin round two of our 2023 Population Health Cohort calls! We look forward to hearing from organizations that have offered to share their insights across key population health initiatives. We will also be bringing our latest and greatest tools and resources your way. We hope you'll join us to network and share strategies to close out 2023 strong!

Cohort Calls are a great opportunity to connect with the Population Health staff at Signify Health, and your ACO peers. In these calls, our staff will lead discussions on specific topics to share best practices, connect with other healthcare professionals, and ultimately meet your patient care goals.

These calls are recommended for Population Health Nurses, ACO Champions, and Practice Managers. Select which Cohort Calls you would like to attend. Topic details, dates, times, and registration links can be found below.

Annual Wellness Visits: Closing Out the Year Strong

Finish out Q4 strong with AWV visits to meet your end-of-year goal (which may include catching up from prior months). In this course, you will have the opportunity to: 

  • Review Coach AWV Opportunities list enhancements and EHR interface feed uses
  • Discuss how foundational programs such as standing orders, team huddles, pre-visit planning, and team-based documentation can improve the efficiency of the visit

Q: Do I have to attend all the Cohort Calls?

A: No, you may pick and choose the calls you would like to attend. You must register separately for each call you would like to participate in.

 

Q: Will session recordings be available on-demand?

A: Session recordings will be available on Signify Health Learn at a later date, but we encourage you to attend live for the best learning and networking experience.

 

 

Learning Environment

Our events are an open forum for individuals to learn and network with their peers. Participants consistently report that the greatest benefit comes from hearing what others are doing and sharing their own experiences. Signify Health reserves the right to ask individuals who disrupt this learning environment to leave the calls.
