What should I expect from my visit?

Before your visit

Schedule your In-Home Health Evaluation online through the scheduling website, or by phone. After you have made an appointment, you may also get:

  • Appointment confirmation
  • Email, text or call reminder 24 hours prior to appointment

During your visit

On the day of your visit, a licensed clinician will spend one-on-one time discussing your health in the comfort of your home. They’ll answer your health questions and more including:

  • Physical examination
  • Reviewing medical history
  • Going over medications
  • Performing diagnostic and preventative screenings (as needed)

After the visit

We’ll share a summary of the visit with you and your primary care provider, if you have one. We may also help coordinate other services or connect you with resources you need.

Why is Signify Health calling me?

Signify Health works with leading health plans, offering no-cost, In-home Health Evaluations to plan members. Once a health plan identifies people who might benefit from an In-home Health Evaluation, a member of the Signify Health team contacts the member to schedule an in-home visit. These visits are then confirmed and performed by a Signify Health-contracted clinician, like a doctor or nurse practitioner.

Are In-Home Health Evaluations available on nights and weekends?

What does an In-Home Health Evaluation cost?

As a potential part of your health plan benefits, the In-Home Health Evaluation is offered at no additional cost to you.

Does Signify Health work with my insurance plan?

Signify Health is a trusted partner of many leading health plans. In fact, we partner with at least 24 out of the top 50 Medicare Advantage plans, as well as providing services to certain Medicaid and commercial plan members. How can I find my results from my In-Home Health Evaluation?

You will receive a one-page summary and action plan from your Signify Health clinician immediately after your visit. A full report on your visit is also securely shared with your primary care provider, if you have one. And you can always Signify Health’s Member Services team, by calling 855-319-448, with questions about your recent visit.
