At Signify Health, a CVS Health® company, our mission is to build trusted relationships to make people healthier. Our national network of more than 12,000 clinicians works to help people gain a more comprehensive view of their health by conducting In-Home Health Evaluations (IHEs). During an IHE, clinicians identify chronic conditions, close gaps in care, and address social determinants of health, making it possible for health plan members to get connected to the right care for their unique needs.
We’re proud to showcase the clinicians helping us build a more connected, effective care experience for all – and their personal stories that make this important work feel #ClosetoHome. This is Dahnide’s story.
Too often, people pay attention to their health after a serious diagnosis or health care complication. For Dahnide Taylor-Williams, the most important stage of health care occurs earlier, when complications or conditions can be prevented. Those values are what led Dahnide to nursing and to Signify Health.
“I believe prevention is so important. With Signify Health, we’re delivering a real value to the health plan member. We visit their homes and assess them where they are. I want to help people before something major happens to their health, whether that’s preventing a condition from developing at all - or heading off complications before they become more serious,” Dahnide said.
After earning a master’s in public health, Dahnide continued her health care education and became a nurse practitioner. She didn’t want to work in a clinic setting, so a friend recommended Signify Health.
“It’s a great fit. I complete visits based on what works for my schedule. Signify Health is so supportive. When I call with a concern or question, they really listen and provide the support I need,” she said.
While individual needs vary, she frequently encounters members who do not understand their medications. When members express uncertainty about prescriptions, she reviews their diagnosis history, asks questions about recent medical appointments and does her best to answer questions.
Dahnide appreciates the time she spends with health plan members, which may often be longer than what most people have with their regular providers. Those conversations - and the gratitude members express - makes her role worthwhile.
“Whether they compliment me on the way I answered their questions or say ‘I’m glad you came,’ I feel good about my job,” she said. “Those positive experiences help you move along through the day and help the next person.”
Here’s one such example, as relayed to Signify Health’s Member Services team.
“Nurse practitioner [Dahnide] was great. She came on time to my time, thoroughly reviewed my medication and health measures and gave recommendations to ask my doctor. She was very professional and knowledgeable.” - Health Plan Member, August 2024
In addition to her work with Signify Health, Dahnide is helping to train the next generation of nurses. As a Clinical Nursing Instructor at the Emory School of Nursing, she enjoys sharing her knowledge with nursing students.
“It’s so rewarding to watch students learn and mature over time. I have a chance to share my knowledge and my journey with them,” she said.
In her free time, Dahnide enjoys traveling, both within and outside of the U.S. Recently, she’s enjoyed trips to Boston, MA to spend time with her niece and trips to other Northeastern states.
