Before your visit
Get the most out of your annual home health visit by taking a few steps to prepare.
Have your health insurance card, medical history and any current lab results from recent visits handy for review.
Gather all of the medications you are taking, including prescriptions and over-the-counter drugs.
Write down any questions you may want to ask during the visit.
For in-person visits
Our licensed clinicians will travel to your home to meet with you, review your medical information, medications, check vitals, and may perform perform preventive and diagnostic screenings. Please take the following steps before your visit:

Designate a quiet place in your home to conduct your health visit.

Have all paperwork and medications ready for review.

Make sure pets are put away so they don’t interfere with the appointment.
For video visits
Online video visits provide a valuable opportunity to speak one-on-one with a Signify Health clinician in a safe, no-contact environment. To get the most out of your appointment, be sure to follow these tips:

Before your visit, be sure to verify that your device has video capabilities.

Make sure your computer, tablet, or smartphone is charged or plugged in.

Find a quiet place to conduct the video call with a strong internet connection.

Have all paperwork and medications ready for review.

After your visit
Following your In-home Health Evaluation visit, you will receive a personalized care plan to share with your primary care provider (PCP). Results from your in-person or virtual visit will also be shared with your PCP to help you and your doctor better assess your healthcare needs at your next appointment.
