Close to Home: Meet Stacy Rohr, Signify Health Nurse Practitioner

During an IHE, clinicians identify chronic conditions, close gaps in care, and address social determinants of health, making it possible for health plan members to get connected to the right care for their unique needs.

Sometimes, meeting health plan members where they are takes clinicians far from home. Recently, Signify Health Nurse Practitioner Stacy Rohr had the chance to escape the heat in South Florida and complete In-Home Health Evaluations (IHEs) in rural Vermont.

“It’s been so interesting to explore a new environment and learn about health care in a different region. The visits have taught me a lot about Vermont’s demographic and their health care system in general,” she said.

Delivering Value with Every Signify Health In-Home Health Evaluation (IHE)

No matter where she completes visits, Stacy sees tremendous value in the IHE. “In the  home setting, we can capture the needs of the whole person and their health. There are some fantastic primary care providers out there, but they don’t always have time to discuss everything,” Stacy said.

She relies on a few strategies to make every IHE as meaningful as possible to the health plan member:

Avoid cancellations with a reminder call: Stacy takes time to call the member the day before the appointment. This helps ensure the member is ready for the visit and reduces questions on the day of the visit.

Identify small steps: Stacy frequently encounters members struggling with obesity. She takes a gentle approach to these conversations, focusing on the benefits and encouraging members to start small.

“I encourage them to find physical activity that works for them, whether it’s a walk or chair yoga. I suggest they start with just five minutes of physical activity and tailor their exercise to what they can realistically do. I make it feel attainable.”

Help the member feel at ease: Stacy knows she has limited time to build trust with the member. She finds simple steps, such as complimenting something in their home, go a long way toward helping members feel comfortable.

“I try to put myself in their shoes. I’m a stranger coming into their home to ask a lot of personal questions. I explain the visit and how it can help them. Once they understand, they become more comfortable.”

Love what you do: If you enjoy your job, it shows. Showing you genuinely care and want to help helps to inspire trust with the health plan member.

Making a Difference and Connecting People to Care
Stacy’s commitment to making a difference started early in her life as a caregiver for her grandmother. She quickly saw that becoming a nurse aligned with her values and what mattered to her.

Stacy came to Signify Health two years ago after working in a private practice. The stress of caring for patients during the pandemic coupled with her interest in preventive care sparked her interest in finding a new path.

“There’s not always enough time during the office visit to talk to patients about what’s really important. That’s what I have time to do now with Signify Health,” she said.

Recently, Stacy encountered a health plan member with untreated atrial fibrillation (AFib). He needed prompt care in the emergency room. Stacy helped him stay calm and connected him to the care he needed.

In her free time, Stacy enjoys working in her garden, traveling, hiking and yoga. In some cases, clinicians have the opportunity to travel outside of their home and adjacent counties to states in which they are licensed.
